Please purchase your ticket for your camping spot for the week of the fair by choosing how many tickets you need and then fill your personal information and card information. Next you will need to read the Terms & Conditions and check the box that you have read and agree. Then you will need to go to the ACYFLS.com and under the EXHIBITOR tab to find Camping form, fill out information about your camper and exhibitors staying in your camper and submit that information. I will email you that I have received the information and that you have completed your registration.

Camping Parental Permission, Release and Waiver of Liability Adult and Minor

Participant:

In consideration of participating in the Alachua County Youth Fair and Livestock Show

Association, Inc. (Hereafter “Releasee”) Activity, I represent that I understand the nature

of the activity and that I am in good health and proper physical condition to participate in

the activity. I acknowledge that if I believe activity conditions are unsafe, I will

immediately discontinue participation in the activity. I full understand that this activity

involves risks, which may be caused by my own actions or inactions, those of others

participating in the activity, the conditions in which the activity takes place or the

negligence of the releasee, and that there may be risks either not known to me or not

readily foreseeable at this time; and I fully accept and assume all such risk and

responsibility for losses, cost and damages I incur as a result of my participation in the

activity. I agree to cooperate and to follow the direction and instruction of the adults

supervising the activity. I agree to cooperate and to follow the direction and instruction

of the adults supervising the activity. I agree that a supervising adult has the right to

dismiss me from the activity if in his or her judgement my actions are inappropriate or

potentially dangerous to myself or to other participants. If my minor child or I are

dismissed, I understand that I will not be reimbursed for fees or costs which I have paid

to releasee for participation in the activity.

Notice to the minor child’s parent or legal guardian, read this form completely and

carefully, you are agreeing to let your minor child engage in a potentially dangerous

activity. You are agreeing that, even if releasee uses reasonable care in providing this

activity, there is a chance that your child may be seriously injured or killed by

participating in this activity because there are certain dangers inherent in the activity

which cannot be avoided or eliminated by signing this for you are giving up your child’s

right and you’re right to recover from releasee in a lawsuit for any personal injury,

including death, to your child or any property damage that result from the risks that are

a natural part of the activity. You have the right to refuse to sign this form, and releasee

has the right to refuse to let your child participate if you do not sign this form. F.S.

Parental consent:

And I, the minor’s parent/legal Guardian, understand the nature of the above referenced

activity and the minor’s experience and capabilities and believe the minor to be qualified

to participate in such activity. I hereby agree to the above and further agree to indemnify

and hold harmless the releasee from all liability, claims, demands, losses or damages

on the minor’s account cased or alleged to have been caused in whole or part by the

negligence of the releasee or otherwise, including negligent rescue operations, and

further agree that if, despite this release , I, the minor, or anyone on the minor’s behalf

makes a claim against the releasee, I will indemnify and hold harmless the releasee

from any litigation expenses, attorney fees, loss liability damages, or costs releasee

may incur as the result of any such claim. I agree the laws of the state of Florida shall

govern the validity of this releasee, the construction of its terms, and the interpretation

of the rights and duties hereunder.
